Either I've been really out of it for the past 2 months or this was changed without any announcement.
There is a new version of the 1696 dated 5/08 with "Destroy Prior Editions" at the bottom.
I am not able to provide a direct link but here is the link to the SSA website form search section - http://www.socialsecurity.gov/cgi-bin/formsearch.pl
If there is anyone who can convert this to a fillable form, I'll send you some cookies. |

I didn't do a detailed comparison, but nothing on the new version jumps out at me as a major change. Ed Johnson is the king of fillable forms. His interest may depend in part on what sort of cookies you are talking about (internet or edible). |

| I did review the form and I notice one major change, and one typo. The change is the ability of the client and representative to authorize release of information to office staff of the representative (see block in Part I). The typo is the statement in Part II (above Rep's signature) that says the "Completion of Part II satisfies this requirement" (to notify SSA of waiver of fee). That statement should instead refer to Part III, not to Part II. Otherwise I see no major changes and I highly doubt that SSA would have a problem accepting the previous version of the form. |

| I intend to switch to this new 1696 ASAP because it looks like it will remove the need for the Form 3288 (the "please talk to my staff too" form) - one less form for every case! |

I don't think that you missed anything. The earliest updates to SSA's internal intranet I could find addressing the new revision were dated 07/17/2008 to 07/20/2008, so they probably only just got the form changes approved and released.
I emailed an updated form to both Mike Walters and David Traver. If it tests out OK I'm sure one or both will make it available shortly. _________________

| Ed even fixed the typo on the form. Only David can upload documents and I would expect he will do so when he has the time. |
